Course Registration

I take full responsibility for determining whether my child is ready and mature enough to participate. I understand the importance of attending all sessions and arriving on time.

Mindful Beginnings reserves the right to decline or send home any student who is disruptive or poses a risk to themselves or others.

I consent to the use of photos or recordings of my child taken during the program for publicity purposes by Safe Sitter, Inc. and/or the Registered Provider.

I acknowledge that there may be a risk of injury involved in course activities. I agree to release, hold harmless, and indemnify Safe Sitter, Inc. and Mindful Beginnings and their staff from any and all claims arising from my child's participation.

I have read and understand all terms above, and I execute this agreement voluntarily. I certify that to the best of my knowledge my child is able to safely participate in the program.

I authorize Mindful Beginnings to submit my child's name and address to Safe Sitter, Inc. Safe Sitter, Inc. will not sell, share, or trade this information with other organizations.
