What this class is. Campus Ready: Safety Skills for College Life is a brief, hands-on safety class taught by a registered nurse. It is educational only and is designed to build the participant's awareness and confidence to help a friend in an emergency. It is not a certification course and does not certify, license, or qualify the participant in CPR, first aid, opioid overdose reversal or naloxone (Narcan), or any other medical or emergency procedure. Any skill shown or practiced is introductory only, and completing the class does not make the participant a trained, certified, or professional responder.
Assumption of risk. I understand that the hands-on activities in this class, including chest-compression practice on a manikin, carry a risk of injury, and I voluntarily accept all such risks, whether known or unknown.
No duty to act. This class creates no obligation for the participant to act in any situation. Any decision to help, or to use anything learned in this class, in a real emergency is the participant's alone and is made voluntarily and at the participant's own risk. The participant should always call 911 and defer to professional responders. Mindful Beginnings LLC is not responsible for anything the participant does or does not do, or for any outcome.
Release and indemnification. In exchange for participation, I, the undersigned (the participant if eighteen years of age or older, or the parent or legal guardian if the participant is under eighteen), release, waive, hold harmless, and indemnify Mindful Beginnings LLC and its owner, instructors, employees, contractors, and agents from any and all claims, demands, liabilities, costs, and causes of action arising out of or related to participation in this class or to any later use of or reliance on anything learned in it, except to the extent caused by gross negligence or willful misconduct.
Emergency care. I authorize the instructor and Mindful Beginnings LLC to arrange emergency medical care for the participant if it becomes necessary during the class, and I accept financial responsibility for that care.
Refund and transfer policy. All sales are final. If a participant is unable to attend, you may transfer the registration to another student. Please email lindsay@mindfulbeginnings.org with the new participant's name. Any payment settlement for the spot must be handled directly between the involved parties.
Course cancellations. Mindful Beginnings reserves the right to cancel any course that does not meet its minimum enrollment requirement. If a course is canceled due to low enrollment, registered families will be notified at least five (5) days before the scheduled date and will receive a full refund.
I have read this release and understand all of its terms. I execute it voluntarily and with full knowledge of its meaning and significance, and by submitting this registration form I provide my signature as proof of acceptance.
